Dear Women’s Healthcare, P.C. patient,

Effective August 1, 2010, I am relocating my practice to The Gabrielson Clinic…for women in Webster City, Iowa. This is an opportunity for me to return to obstetrics after a three year hiatus. I will practice unlimited obstetrical care, including high risk obstetrics, as well as offer total office and surgical gynecological services. I am joining Dr. Dan Gabrielson, one of the top gynecologic surgeons in Iowa.

Thank you to all of you that have supported me over the past 27 years and over 6000+ deliveries. You have been loyal, some of you for the entire 27 years. I have been most fortunate to have been given this vocation and the opportunity to serve you.

I will continue to see patients in Cedar Rapids until 7/30/10. The office will remain open until 10/01/10 for billing and record concerns. Here is a link to a Records Transfer Request. This form allows us to send your records to the physician that you choose for continued gynecological care. Please complete this form and return it to our office at your earliest convenience. You may print it out and send it to us via fax to 319-362-1448, or print it out and mail it to Women’s Healthcare, P.C. at 1260 3rd Avenue SE, Cedar Rapids, IA 52403.
